About the Role
The Social Worker will play a crucial role in supporting patients and their families by providing emotional, psychological, and logistical assistance throughout their journey with palliative care. This individual will be responsible for counseling, advocacy, and community engagement to ensure holistic care for patients while strengthening connections with caregivers and community stakeholders.
Key Responsibilities:
Patient & Family Support:
- Provide emotional and psychosocial support to patients and their families, ensuring their well-being and comfort.
- Conduct patient and family counseling sessions to help them navigate the challenges associated with terminal illness and palliative care.
- Guide families on available medical, financial, and social support resources.
- Facilitate discussions around end-of-life care preferences, advance directives, and decision-making processes.
Community & Stakeholder Engagement: - Build relationships with local community organizations, NGOs, and government bodies to enhance access to resources for patients.
- Conduct awareness programs to educate the community on palliative care and available support systems.
- Serve as a liaison between the centre and external organizations to foster collaboration and improve patient outreach.
Patient Advocacy & Resource Mobilization: - Advocate for patients’ rights, ensuring they receive appropriate care and support.
- Identify and connect patients with relevant government schemes, financial aid programs, and non-profit assistance.
- Work with the administrative and fundraising teams to mobilize resources for patients in need.
Volunteer & Support Group Coordination: - Build and manage a network of volunteers to support patients and their families.
- Facilitate support group meetings for patients, caregivers, and bereaved families.
- Organize community drives and social initiatives to enhance patient well-being.
Documentation & Case Management: - Maintain detailed case histories, patient records, and progress reports.
- Conduct assessments to determine the social, financial, and psychological needs of patients.
- Ensure confidentiality and ethical handling of patient information.
Qualifications & Skills
Educational Background: - Bachelor’s or Master’s degree in Social Work, Psychology, or a related field.
- Training in counseling, palliative care, or healthcare social work is an advantage.
Experience: - 1-2 years of experience in social work, counseling, or patient care. Experience in the healthcare or palliative care sector is preferred.
Language Proficiency: - Fluency in Kannada and English is mandatory.
Skills Required: - Strong interpersonal and counseling skills.
- Empathy, patience, and the ability to handle emotionally challenging situations.
- Knowledge of healthcare systems, government schemes, and community resources.
- Ability to work collaboratively with medical professionals, patients, and caregivers.
