    Job Description

    • Manage a clinical caseload of palliative patients.
    • Lead and develop empowering patient focused therapeutic groups.
    • To support physiotherapy practice in a variety of care settings as part of the wider patient/carer support team.
    • Contribute to the development of the expert patient & carer model being developed to support the needs of its community.
    • To work with the education department on delivering quality education in Palliative rehabilitation to the wider clinical community.
    • Acts as a role model demonstrating the highest standards of enabling palliative rehabilitative care and providing clinical leadership to others to do the same.
    • Promotes a culture that supports self-management independence and embeds patient and community involvement within the sphere of practice.
    • Contributes to and delivers specialist education and training to other professionals involved in patient care. Contributes to the development of patient and community focused education, including coaching patients and their families and carers.
    • Contributes to the development, management and promotion of the service ensuring the delivery of high quality rehabilitative palliation
    • Collates and evaluates data to provide evidence of productivity, outcomes and quality.
    • Accountable for professional decision making.
    • Participates in service review in order to drive service improvement.
    • Contributes to the implementation and monitoring of adherence to clinical guidelines.
    • Makes sure that people with life-limiting illnesses are aware of the full range of resources and services available.
    • Working closely with statutory Physio services.
    • Demonstrates skills in conflict resolution and competent negotiation skills when dealing with difficult or challenging situations such as managing patient and professional unmet expectation.

    Job Description

    • As a Palliative Care Physician provide symptom management, pain relief and supportive care for the patient in inpatient care, outpatient clinic and home based care.
    • Facilitate clarification of patient and family goals of care.
    • Facilitate information sharing and decision making regarding patient care.
    • In addition to providing expert symptom management, as a palliative care physician you need to devote time to intensive family meetings, ensure coordinated care across health care settings, link patients and family members to support groups and other services and improve access to information so patients can make confident, well-informed decisions about their care.
    • Consult with fellow physicians and the interdisciplinary team in establishing a written plan of care at intervals specified in the plan.
    • Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.
    • Assist in the development of standards of care and practice.
    • Participate first and organise academic sessions and training programs for skill development. 
    • Facilitate and participate awareness campaigns in the community.

    Job Title: Medical Social Worker

    Qualification: Masters in Social Work (Full-Time) specialised in Medical and Psychiatry from a recognized University.

    Experience: Fresher, 1-2 years

    Skill set required

    • Excellent interpersonal skills and desire to work as part of a team.
    • Ability to communicate effectively.
    • Excellent organizational skills and attention to details; competence with management of documentation and paperwork.
    • Flexibility, professional demeanour, ability to respond calmly and positively to rapidly changing situations. 
    • Proficient in Microsoft Office.
    • Proficiency in English and Malayalam language.

    Responsibilities & Duties

    • Evaluate and devise plans of care for patients and families requiring palliative care, with particular attention to psycho-social and socio-economic issues.
    • Be part of the inter-disciplinary palliative care team and contribute to the plan of care for patients and their families vis-à-vis:
      • Thorough and competent assessment of patient + family history
      • Current status, needs and concern
      • Follow-ups
      • Formulating plan of care in conjunction with the rest of the interdisciplinary team
      • Coordination with relevant government/ non-government social support agencies and facilitating access to their services 
      • Providing bereavement support to families. 
    • Audit palliative patient care with the intent to identify problems, formulate possible solutions and bring these to the attention of the administration.
    • Take part in on-going research activities or initiate needs-based research programs with the approval of scientific committee.
    • Liaison with the appropriate government departments to access government pensions, subsidies and various schemes of assistance. 
    • Publish reports/data from audits or research programs with the objective of improved patient care
    • Send regular updates to the reporting authority and other superior officials.
    • Attend (first) and organize (later) educational programs on behalf of Pallium India.
    • Maintain good relations with clients, donors, well-wishers and staff during telephone conversations, email contacts, personal contacts or any other.
